Mediator throwing - Transport Out Info has not been set error in transform service

When Centrasite virtual service calls Mediator transform service, Mediator is throwing the below error while sending the response out.

Mediator encountered an error:Transport out has not been set while executing operation:{Domain im Kundenauftrag registriert}MT_ItemListingData_OAG service:VS_R10RangingMaintenance at time:5:20:15 PM on date:05/06/2014. The client ip was:10.138.35.159. The current user:Default. The consumer application:null

Any configuration missing that needs to be set

Could you share the VSD and which version of centrasite / mediator are you using.
